1. PERSONAL DATA WE PROCESS
We process your personal data because you use my services and/or because you provide this information to me yourself. The digital file also contains short notes/keywords about relevant information you provided to me for the treatment. Only Sanne Homewood has access to the data in your file. First and last name / Date of birth / Phone number / Email address.
For proper treatment it is necessary that I create a file with key words about what was discussed during the consultation with regard to the problems you experience, the goals you want to achieve and important events in your life. In addition, it is noted which interventions have been made. This is also a legal obligation imposed by the WGBO.
The file also contains information that is necessary for your treatment, and which I will only request from another healthcare provider such as a general practitioner after your explicit permission. I have a legal duty of confidentiality (professional secrecy) and I do my best to guarantee your privacy.
This means, among other things, that I:
– handle your personal and any medical data with care,
– make sure that unauthorized persons do not have access to your data
2. THE DATA FROM YOUR FILE CAN ALSO BE USED FOR THE FOLLOWING PURPOSES
– To inform other care providers, for example when the therapy has been completed or when a referral is made to another practitioner. This only happens with your explicit permission.
– For use for observation, during my absence, provided this has been agreed with you.
– For the anonymized use during peer review and supervision.
– A small part of the data from your file is used for the financial administration, so that I can draw up an invoice. If I want to use your data for another reason, I will first inform you and explicitly ask for your permission.
3. SPECIAL AND/OR SENSITIVE PERSONAL DATA THAT WE PROCESS
My website and/or service does not intend to collect data about website visitors under the age of 16. Unless they have parental or guardian consent. However, we cannot check whether a visitor is older than 16. We therefore advise parents to be involved in the online activities of their children, in order to prevent data about children from being collected without parental consent. If you are convinced that we have collected personal information about a minor without this permission, please contact us at sannehomewood@gmail.com and we will delete this information.
